Hospital Discharge

We pride ourselves in providing care to our clients that have been discharged from hospital on a care package. We also make assessments on how they manage their physical activities such as moving around the house and their overall mobility. We assess/monitor their mental status, for example memory, orientation and communication just to name a few.
